Always use the least effective dose to get the most return.
Receptors will sensitive to low dose, provided there has been a clearance time to allow down regulation of the receptor.

With insulin sensitivity being an issue, I don’t think supplementing with insulin is going to have much benefit if receptors are not responding. Hence why metformin is normally given in these cases to increase sensitivity to endogenous insulin.
